In this case we will need to see documents that show the date of and reason for your name change.
For married women we will need to see marriage certificates.
For civil partners, we will need to see either your civil partnership certificate or dissolution certificate.
If you have gone back to your maiden name we will need to see your birth certificate and a signed statement from you to say that you have gone back to your maiden name for all purposes.
In all other cases we need to see your change of name deed. And if you have changed your name more than once we need to see evidence of all changes of name.
You must send us original copies of your documents. We do not accept photocopies of documents or documents that have been laminated. We do however accept laminated documents for change of name purposes only.
