I want to apply to amend my passport (change my name)

When you apply please be sure to include:

completed application form*
two identical passport photos
form PD2 if you are waiting to get married
evidence of any past name change(s)
your birth certificate and a signed statement if you are reverting to your maiden name
your current or last passport
payment.

*Note: if you use the online application form, we will print it out and send to you to sign. When you post it back to us you should also send your own/your child’s documents and photos.

And remember, when your passport arrives, check it carefully. Please make a note of the passport number or take a photocopy of the page showing your details and keep this information in a safe place.
