We have one standard form that you can use for all types of individual applications.

How do I get it?

You can:

Collect it from one of our Check & Send outlets (selected Post Office® branches). Find my nearest Check & Send outlet.
Fill in the online application form request and we will send you a form in the post.
Call the 24-hour Passport Adviceline 0300 222 0000 (see terms and conditions)
Go to the online application form.

How do I complete it?

The form comes with clear and detailed instructions, whether you fill in the paper version or complete it online. Please follow these carefully as any errors will delay your application.

For the paper form it is important to:

complete the form in CAPITAL LETTERS with a BLACK ballpoint pen
write only within the white boxed areas
leave other areas of the form blank (do not strike through or otherwise mark them).

Who needs to sign it?

You need to sign the form yourself and should also get it countersigned at section 10 by someone who works in a recognised profession or has good standing in the community (tell me more about who can countersign my application form) who has known you for at least two years. They must:

be over 18
hold a current British or Irish passport
be willing to write their passport number on the form
certify, sign and date one of your photographs.
