We will need to see documents that show the date of and reason for your name change.
For married women we will need to see your marriage certificate.
For civil partners, we will need to see your civil partnership certificate.
If you have gone back to your maiden name we will need to see your birth certificate and a signed statement from you to say that you have reverted to your maiden name for all purposes.
In all other cases we need to see your change of name deed. And if you have changed your name more than once we need to see evidence of all changes of name
You must send us original copies of your documents. We do not accept photocopies of documents or documents that have been laminated. We do however accept laminated documents for change of name purposes only.
