There is a special application form for collective passports. You can get a copy in three ways:
The electronic version is faster and more secure. You should save it to your own computer, complete it* and email it directly to your local passport office. Once you have e-mailed your application form you must print off a hard copy. This hard copy must be signed by the Leader and the Deputy Leader. You must send us this signed paper copy of the application form along with the fee and other supporting documents we need.
You should submit the application form, fee and supporting documents at least four weeks before the passport is needed. You should allow six weeks if you are applying between January and July, when demand is greatest.
* Note: The electronic version of the application form must be completed electronically and e-mailed to your local passport office. Electronic forms, which have been downloaded and completed by hand, will not be accepted.
