I dderbyn ffurflen LS01 rhowch y manylion a ganlyn a’u cyflwyno, gan sicrhau eich bod yn llenwi POB adran a nodir gyda seren (*). Os rhowch eich cyfeiriad e-bost inni, byddwn yn anfon neges atoch i ddweud ein bod wedi derbyn eich cais.
The Identity and Passport Service is only able to issue and service UK passports to British nationals who are resident in the UK at the time of application. If you are a British national and do not live in the UK currently, you can apply for a replacement passport at your nearest British embassy, high commission or consulate. Alternatively, if you are currently visiting the UK you may apply in person for a passport. In order to do this, you need to make an appointment at a passport office and you must provide an address in the UK to which the passport may be posted. You should call our 24-hour Passport Adviceline 0300 222 0000 (see terms and conditions) to make an appointment at one of our offices. We can offer a Fast Track one week service. Please note that we do not accept applications by post or e-mail if you live abroad.
