This page should be fully completed with the details of the school or organisation, full details of the trip, the Leader and Deputy Leader’s details, with their names as they appear on their current passport. The Leader and Deputy Leader must sign the declaration.

List of members of the party – the list of members of the party must be completed in alphabetical surname order, with first name, town and country of birth* (in full), date of birth and town of residence*. You might find the example below helpful.

*Note: You may abbreviate country of birth to UK (where applicable). The town of residence must appear in full, for example, Stoke on Trent or Newcastle upon Tyne.

Note: for postal applications, page 2 of the application form will be part of the official collective passport. For this reason please do not:

use ditto marks
use abbreviations other than ‘UK’
make alterations/crossings out
use correction fluid (e.g.Tippex)
