I want to apply for a collective (or group) passport

Before you send us your application please check you have enclosed the following:

The paper/hard copy of the application form if you are applying electronically. Please ensure this has been signed by Leader and Deputy Leader and that the electronic version of the form has been e-mailed to your local passport office
Nationality questionnaire and parental consent forms (CPNQ) for every child included on the collective passport
Nationality questionnaire and parental consent forms (CPNQ) for every child included on the collective passport
Collective passport photo cards for every child. Please ensure all the cards are completed correctly and have been certified by the Group Leader
